Ah ha.  Now I being to see the problem.  patch should remove those files 
during patching, but doesn't by default.  Use "patch -E" when applying 
the patches.

Pager (under some configurations) will write a ".small" file, which 
contains a smaller version of the pixmap displayed in pager.  This file 
is an xpm, no matter what the format of the desktop background image 
normally is.
